Evidence mapping worksheet
Connect evidence, criteria and actions. Export an editable CSV.
Give every evidence gap an owner.
Use the official plan’s exact outcome or criterion reference. Do not paste apprentice names, client data or confidential evidence. This worksheet does not verify evidence sufficiency.
| Outcome / criterion | Evidence reference | Owner reference | Next action | Remove |
|---|---|---|---|---|
Up to 50 rows. CSV cells that could be interpreted as spreadsheet formulas are escaped.
Private by design: entries are processed in this browser and are not sent to Avenor. Exports may contain what you type; use learner reference codes rather than personal or sensitive data.
How to use it
Start with a real planning task.
Use this to turn a plan criterion into an evidence reference, a named internal owner and a next action before files are transferred through an approved route.
- Copy the exact outcome or criterion reference from the applicable plan.
- Describe the permitted evidence by reference, without pasting its sensitive contents.
- Assign an owner reference and next action, then export the map to your authorised record system.
Worked example
Reproduce the result.
- Input
- Outcome / criterion: REF-01 (synthetic, not a published KSB); evidence reference: PR-04; owner reference: TP-QA; next action: confirm apprentice contribution.
- Output
- CSV row: REF-01 (synthetic, not a published KSB) | PR-04 | TP-QA | confirm apprentice contribution.
Interpretation and limits
Know what the output can support.
Coverage in a worksheet does not establish authenticity, sufficiency or assessment eligibility. A qualified reviewer must inspect the permitted evidence in context.
Does the tool upload evidence?
No. It runs in the browser and exports text references only. Use the approved secure route for evidence files.
Can one item cover several criteria?
Record each claimed link explicitly and be ready to explain the apprentice’s contribution. Reuse does not prove sufficiency.
Who decides whether evidence is enough?
The accountable human reviewer working under the applicable plan and assessment process.
